1.That I was born on 02-05-1959 (2nd day of May 1959) at Village Kalyanpur, District Amarnagar.
2.That I do not have a birth certificate because my birth was not registered at that time.
3.That my date of birth stated above is correct, and I am making this affidavit for its use in my records.

DEPONENT
What to bring
Papers to bring
- Aadhaar card or another photo identity proof (photocopy; the original to show)
- Any record of your date of birth: the matriculation or school leaving certificate, passport, PAN card, voter card, or a service or insurance record
- A non-availability certificate of birth from the Registrar, if the office asks for one
- Two recent passport-size photographs, if the office asks for them
Where to go
Sworn before an Oath Commissioner or Notary, then given to the office that needs it: the passport office, a bank, an employer or the tehsil.
Fee
The e-Stamp paper and the Oath Commissioner's or Notary's fee are paid at the counter; ask there for today's amounts.
Good to know
- The date must agree with your other records, above all the matriculation certificate and Aadhaar; a difference will usually be questioned.
- More and more offices want the birth certificate itself; a birth that was never registered can still be registered late (the app has an affidavit for that).
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
Stamp paper
Punjab: affidavits are usually typed on e-stamp paper (buy it at an authorised e-stamp centre; check the current duty there) and sworn before an Oath Commissioner or Notary. Many state services now accept a self-declaration instead.
Haryana: use e-stamp paper (egrashry / authorised vendors; check the current duty) and swear before an Oath Commissioner or Notary.
Chandigarh: e-stamp paper from an authorised centre (check the current duty); sworn before an Oath Commissioner or Notary.
