(Correct address)
I, Rohan Kumar son of Shri Om Prakash, aged about 37 years, resident of House No. 12, Street No. 3, Nanak Nagar, Amarnagar, Tehsil and District Amarnagar, Punjab, do hereby solemnly affirm and declare as under:
1.That I live at the address given above with my family.
2.That My earlier address in the electricity bill was House No. 21, which was renumbered as House No. 12 by the Municipal Committee.

DEPONENT
What to bring
Papers to bring
- Aadhaar card or another photo identity proof (photocopy; the original to show)
- Copies of the papers your paragraphs refer to, marked as annexures; the originals to show
- For a court case: the case number, the names of the parties and the next date
- Two recent passport-size photographs, if the office asks for them
Where to go
Sworn before an Oath Commissioner (at the district courts) or a Notary. For a court case, the advocate files it in that case; otherwise give it to the office that asked for it.
Fee
The e-Stamp paper and the Oath Commissioner's or Notary's fee are paid at the counter; an affidavit made only to be filed in a court is usually free of stamp duty. Ask there for today's amounts.
Good to know
- State only facts you know yourself; where something comes from papers or from what others told you, say so in that paragraph.
- Come in person with the original identity proof: the Oath Commissioner or Notary must see you sign. A false affidavit is an offence.
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
Stamp paper
Punjab: affidavits are usually typed on e-stamp paper (buy it at an authorised e-stamp centre; check the current duty there) and sworn before an Oath Commissioner or Notary. Many state services now accept a self-declaration instead.
Haryana: use e-stamp paper (egrashry / authorised vendors; check the current duty) and swear before an Oath Commissioner or Notary.
Chandigarh: e-stamp paper from an authorised centre (check the current duty); sworn before an Oath Commissioner or Notary.
