What to bring
Papers to bring
- Aadhaar card or another photo identity proof (photocopy; the original to show)
- The police report of the loss (the lost article report or its number), or the FIR if the papers were stolen
- A photocopy of each lost document if you have one, or its number, its date and the office that issued it
- The newspaper cutting of the loss notice, if the office asks for it
- Two recent passport-size photographs, if the office asks for them
Where to go
Sworn before an Oath Commissioner or Notary, then given with the application for a duplicate to the office that issued the document: the school board or university, the transport office, the bank, or the Sub-Registrar for a certified copy of a registered deed.
Fee
The e-Stamp paper and the attestation fee are paid at the counter; the issuing office charges its own fee for the duplicate, and the newspaper notice is paid separately. Ask for today's amounts.
Good to know
- Report the loss to the police first; in Punjab, Haryana and Chandigarh this can usually be done online on the police website, and the report number goes into the affidavit.
- If the papers were stolen, report a theft (FIR), not a loss.
- If the original turns up later, tell the office and return the duplicate, as the affidavit promises.
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
Stamp paper
Punjab: affidavits are usually typed on e-stamp paper (buy it at an authorised e-stamp centre; check the current duty there) and sworn before an Oath Commissioner or Notary. Many state services now accept a self-declaration instead.
Haryana: use e-stamp paper (egrashry / authorised vendors; check the current duty) and swear before an Oath Commissioner or Notary.
Chandigarh: e-stamp paper from an authorised centre (check the current duty); sworn before an Oath Commissioner or Notary.
