What to bring
Papers to bring
- Aadhaar card or another photo identity proof (photocopy; the original to show)
- The marriage certificate, or the wedding card and photographs if the marriage is not registered
- A record in your name before marriage, such as the matriculation certificate (photocopy)
- Your husband's identity proof (photocopy)
- Two recent passport-size photographs
Where to go
Sworn before an Oath Commissioner or Notary. Many offices then also want the change published in a newspaper, and some in the Gazette; ask the office where the record is to be changed.
Fee
The e-Stamp paper and the attestation fee are paid at the counter; newspaper and Gazette charges are separate. Ask for today's amounts.
Good to know
- For a passport, a woman taking her husband's surname usually needs only the marriage certificate, or the joint affidavit of husband and wife with photographs (in the app).
- Keep the newspaper cutting and a copy of this affidavit: banks, the school board and other offices ask for them.
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
Stamp paper
Punjab: affidavits are usually typed on e-stamp paper (buy it at an authorised e-stamp centre; check the current duty there) and sworn before an Oath Commissioner or Notary. Many state services now accept a self-declaration instead.
Haryana: use e-stamp paper (egrashry / authorised vendors; check the current duty) and swear before an Oath Commissioner or Notary.
Chandigarh: e-stamp paper from an authorised centre (check the current duty); sworn before an Oath Commissioner or Notary.
