1.That we are husband and wife. Our recent photographs are pasted below, and each of us has signed across his or her own photograph.
2.That we were married to each other on 08-02-2015 at Amarnagar, according to Hindu rites and ceremonies, and since then we have been living together as husband and wife.
3.That we do not have a certificate of registration of our marriage, because our marriage was never registered, and we make this joint affidavit in its place.
4.That at the time of the marriage neither of us had a living spouse. Our marriage subsists: it has not been dissolved or annulled, and no proceeding for divorce, judicial separation or nullity is pending between us.

Good to know
- Both spouses usually have to sign together, in person; carry the originals of everything to the Passport Seva Kendra.
- Check the current format on the Passport Seva website before printing; it changes from time to time.
- If the marriage is registered, the marriage certificate usually does instead of this affidavit.
