What to bring
Papers to bring
- Aadhaar card or another photo identity proof (photocopy; the original to show)
- Details of what you consent to: the person, the act, the dates and any conditions
- Proof of your right to consent, such as ownership papers or proof of the relationship, if the office asks
Where to go
Signed by you and given to the person or office that needs your consent; plain paper is usually enough. Some offices want it witnessed or attested by a Notary.
Fee
No fee on plain paper; if the office wants a Notary, the Notary's fee is paid at the counter.
Good to know
- Give an end date where you can; to withdraw the consent, write to the same person or office, and keep copies of both.
- For a minor's travel abroad use "Affidavit of both parents for a minor's travel"; hospitals and banks usually have their own consent forms.
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
