What to bring
Papers to bring
- Aadhaar card or another photo identity proof (photocopy; the original to show)
- Details that identify the record: the registration, admission or patient number, the file number and the dates
- For someone else's records, the consent, authority letter or proof of relationship the office asks for
Where to go
Given at the office, school or hospital that keeps the record, addressed to the officer in charge; get the receiving with the office stamp and date on your copy.
Fee
No stamp paper is needed; the office charges its own fee for copies or certified copies. Ask there for today's amount.
Good to know
- If a public office does not give the records, you can apply under the RTI Act (see "RTI application (s.6(1) RTI Act)").
- Ask how long the copies will take, and follow up with the copy that has the receiving.
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
