AFFIDAVIT
(Non-remarriage)
I, Balwant Kaur widow of late Shri Jagir Singh, aged about 67 years, resident of Village Kalyanpur, Tehsil and District Amarnagar, Punjab, do hereby solemnly affirm and declare as under:
1.That my husband, late Shri Jagir Singh, died on 19-06-2021.
2.That I have not remarried since his death, and I am living as his widow.
3.That I am receiving family pension (PPO No. 12345), and I undertake to inform the office concerned at once if I remarry, and to refund any amount received after that.

What to bring
Papers to bring
- Aadhaar card or another photo identity proof (photocopy; the original to show)
- Your husband's death certificate (photocopy)
- The pension payment order (PPO) or sanction letter, and the passbook of the account the pension comes into
- The bank's or office's own form for the yearly certificate, if it has one
- Two recent passport-size photographs, if the office asks for them
Where to go
Sworn before an Oath Commissioner or Notary, then given to the bank branch or treasury that pays the pension, or to the office named in the sanction letter. Many offices ask for it once a year, often with the life certificate.
Fee
An affidavit made only to receive a pension is usually free of stamp duty; the attestation fee is paid at the counter. Ask there for today's amounts.
Good to know
- Tell the pension office at once if you remarry: the affidavit promises to refund whatever is paid after that.
- Whether a pension continues after remarriage depends on the rules of that pension; ask the pension office rather than guess.
- Write the names and the date of death exactly as in the death certificate and the PPO.
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
Stamp paper
Punjab: affidavits are usually typed on e-stamp paper (buy it at an authorised e-stamp centre; check the current duty there) and sworn before an Oath Commissioner or Notary. Many state services now accept a self-declaration instead.
Haryana: use e-stamp paper (egrashry / authorised vendors; check the current duty) and swear before an Oath Commissioner or Notary.
Chandigarh: e-stamp paper from an authorised centre (check the current duty); sworn before an Oath Commissioner or Notary.
