What to bring
Papers to bring
- Aadhaar card or another photo identity proof (photocopy; the original to show)
- A copy of the record with the wrong name
- Proof of the correct name: your father's or husband's Aadhaar or other identity proof, and a record of yours that shows it rightly, such as the matriculation certificate or passport
- For a husband's name: the marriage certificate, or the wedding card and photographs
- Two recent passport-size photographs, if the office asks for them
Where to go
Sworn before an Oath Commissioner or Notary, then given with the correction form to the office that keeps the record: the school board, the bank, the passport office, an Aadhaar centre or the tehsil.
Fee
The e-Stamp paper and the attestation fee are paid at the counter; the office that corrects the record may charge its own fee. Ask for today's amounts.
Good to know
- Each office has its own correction form and steps; some, such as the school boards, also want a newspaper notice or a fee for a fresh certificate.
- Correct the main records first (matriculation certificate, Aadhaar, passport): other offices go by them.
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
Stamp paper
Punjab: affidavits are usually typed on e-stamp paper (buy it at an authorised e-stamp centre; check the current duty there) and sworn before an Oath Commissioner or Notary. Many state services now accept a self-declaration instead.
Haryana: use e-stamp paper (egrashry / authorised vendors; check the current duty) and swear before an Oath Commissioner or Notary.
Chandigarh: e-stamp paper from an authorised centre (check the current duty); sworn before an Oath Commissioner or Notary.
