What to bring
Papers to bring
- The birth certificate issued without the name, or the registration number and date
- Identity proof of both parents (photocopies)
- Any record already in the child's name, such as the school admission record or Aadhaar, so that the spelling matches
- For a guardian, the court's guardianship order or other proof of guardianship
Where to go
Sworn before an Oath Commissioner or Notary, then given with the request to the Registrar of Births and Deaths who registered the birth (the municipal body, or the health department's or panchayat's registration unit), usually through the Sewa Kendra, a Saral Kendra or a Sampark centre.
Fee
The Registrar may charge a late fee for adding the name, and each copy of the certificate has its own fee; the e-Stamp paper and the attestation fee are paid at the counter. Ask for today's amounts.
Good to know
- Check the spelling letter by letter before signing: the certificate prints it exactly, and changing it later is much harder.
- A name can be added to an old registration only within a time limit; if the birth was registered years ago, ask the Registrar soon.
- Ask for a few certified copies: the school, the Aadhaar centre and the passport office each want one.
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
Stamp paper
Punjab: affidavits are usually typed on e-stamp paper (buy it at an authorised e-stamp centre; check the current duty there) and sworn before an Oath Commissioner or Notary. Many state services now accept a self-declaration instead.
Haryana: use e-stamp paper (egrashry / authorised vendors; check the current duty) and swear before an Oath Commissioner or Notary.
Chandigarh: e-stamp paper from an authorised centre (check the current duty); sworn before an Oath Commissioner or Notary.
