AFFIDAVIT
(Photograph and identity)
I, Vikram Singh son of Shri Harbans Singh, aged about 52 years, resident of House No. 45, Gulab Colony, Amarnagar, Tehsil and District Amarnagar, Punjab, do hereby solemnly affirm and declare as under:
1.That the photograph on my driving licence issued in 2004 is mine, taken some years ago.
2.That my appearance has changed since then because I now keep a beard and have aged, and my recent photograph is pasted below and attested.
3.That the person in both photographs is one and the same, that is, myself.
DEPONENT
VERIFICATION: Verified at Amarnagar on this 12th day of October 2026 that the contents of paragraphs 1 to 3 of the above affidavit are true and correct to the best of my knowledge and belief, and nothing material has been concealed therein.

What to bring
Papers to bring
- The document with the old photograph (the original and a photocopy)
- Another recent photo identity proof, such as Aadhaar, a voter card or a passport
- One recent passport-size photograph, to paste in the box on the affidavit and have attested across
- The medical record, if the change comes from an illness, an accident or surgery
- Two recent passport-size photographs, if the office asks for them
Where to go
Sworn before an Oath Commissioner or Notary, who attests the photograph by signing or stamping across it and the paper; it then goes to the bank, passport office or other office. Some offices also want the photograph attested by a gazetted officer or the bank manager; ask.
Fee
The e-Stamp paper and the Oath Commissioner's or Notary's fee are paid at the counter; ask there for today's amounts.
Good to know
- Update the photograph in the document itself when you can (Aadhaar at an Aadhaar centre); this affidavit is for the meantime.
- Come in person with the original identity proof: the Oath Commissioner or Notary must see you sign.
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
Stamp paper
Punjab: affidavits are usually typed on e-stamp paper (buy it at an authorised e-stamp centre; check the current duty there) and sworn before an Oath Commissioner or Notary. Many state services now accept a self-declaration instead.
Haryana: use e-stamp paper (egrashry / authorised vendors; check the current duty) and swear before an Oath Commissioner or Notary.
Chandigarh: e-stamp paper from an authorised centre (check the current duty); sworn before an Oath Commissioner or Notary.
