Sir/Madam,
1.That Karnail Singh, son of Bachan Singh, died on 18-01-2024 at his house at Model Town, Amarnagar. The applicant is the son of the deceased.

What to bring
Papers to bring
- The affidavit: "Affidavit for late registration of a birth" or "Affidavit for late registration of a death"
- The Registrar's report that there is no entry in the register (non-availability certificate)
- Proof of the birth or death: the hospital record or school record, or for a death at home the cremation or burial ground receipt
- Identity and address proof of the applicant and, for a birth, of the parents
- Statements of the sarpanch, ward councillor or two respectable persons who know the facts, if the office asks
Where to go
At the office of the SDM or the District Magistrate (Deputy Commissioner), often through the Sewa Kendra (Punjab), a Saral Kendra (Haryana) or a Sampark centre (Chandigarh). After verification the order goes to the Registrar, who registers the event and issues the certificate.
Fee
The late fee under the state's birth and death registration rules, and any service centre fee, paid as the office directs; ask there for today's amounts.
Good to know
- Apply where the birth or death took place, not where the family lives now.
- The office may verify the facts, hear witnesses or publish a notice before the order; keep the originals ready.
- Within a year of the event this order is not needed: the Registrar registers it on the affidavit and a late fee, with any permission the rules require.
