AFFIDAVIT
(Correction of school record)
I, Rohan Kumar son of Shri Om Prakash, aged about 37 years, resident of House No. 12, Street No. 3, Nanak Nagar, Amarnagar, Tehsil and District Amarnagar, Punjab, do hereby solemnly affirm and declare as under:
1.That in the admission register of Government Primary School, Nanak Nagar, for 2022 of Arjun, an entry has been wrongly made as "14-08-2017" instead of "14-03-2017".
2.That the correct entry is shown by the birth certificate, a copy of which is attached.
3.That the mistake is a clerical one, and its correction will not affect anyone's rights.
4.That I request that the entry be corrected to "14-03-2017".
DEPONENT

What to bring
Papers to bring
- Identity proof of the parent or student making the affidavit (photocopy; the original to show)
- The record with the mistake: the school leaving certificate, report card or board certificate (photocopy; the original to show)
- Proof of the correct entry: the birth certificate, Aadhaar, or the parent's identity proof
- The school's or board's correction form, if it has one
Where to go
Sworn before an Oath Commissioner or Notary, then given to the head of the school with the application. For a board certificate (PSEB, HBSE, CBSE or another board), the school usually forwards it to the board, which follows its own procedure.
Fee
The e-Stamp paper and the attestation fee are paid at the counter; a board charges its own correction fee. Ask for today's amounts.
Good to know
- Apply as soon as you notice the mistake: boards often correct a certificate only within a time limit.
- A date of birth is usually corrected only to match the admission record or the birth certificate; later changes are treated strictly.
- Correct the school record first: Aadhaar, the passport and other records usually follow the matriculation certificate.
Papers and fees can change. Check with the office before you go. Guidance only, not yet reviewed by a lawyer.
Stamp paper
Punjab: affidavits are usually typed on e-stamp paper (buy it at an authorised e-stamp centre; check the current duty there) and sworn before an Oath Commissioner or Notary. Many state services now accept a self-declaration instead.
Haryana: use e-stamp paper (egrashry / authorised vendors; check the current duty) and swear before an Oath Commissioner or Notary.
Chandigarh: e-stamp paper from an authorised centre (check the current duty); sworn before an Oath Commissioner or Notary.
